How much time do I need?
Your first visit or evaluation will last approximately 60 minutes and follow up sessions 30 to 45 minutes.
What should I bring to my first visit?

A physical therapy referral is preferred.

A valid ID.

Medicare insurance cards, including supplemental insurance cards.

Come dressed in comfortable and loose fitting clothes to expose the area being treated.
Insurance Requirements
At this time we accept Medicare only however we do offer a cash based or self pay rate:

The initial visit is $140, billed at the time of service.

Follow up visits are $75, billed at the time of service.
Paperwork
Let’s get the paperwork out of the way! You can download the patient intake forms and bring it to your first visit. If you are unable to print these forms, don’t worry, we got you covered. Just arrive 15 minutes early to fill them out.
Privacy Policy
We respect your privacy and confidentiality and all patients will be provided with our HIPPA policy at the time of the evaluation.
